NP / RN / PA Aesthetic Practice FAQ
How much do Botox and filler cost wholesale for a medspa?
Wholesale (cost-to-medspa) pricing in 2026: Botox (Allergan) typically $4-$5 per unit; Dysport (Galderma) typically $1.20-$1.60 per unit; dermal fillers (Restylane / Juvederm / RHA) typically $200-$350 per 1mL syringe. Prices vary by volume tier, account loyalty programs, and current promotional cycles.</a>
Wholesale pricing in aesthetic injectables shifts frequently based on manufacturer promotional cycles, volume tier achievement, and loyalty program rewards. Ranges below are 2026 typical bands for the US market; verify with your specific account representative.
Neuromodulators (cost per unit of toxin):
- Botox (Allergan / AbbVie): typically $4.00–$5.00 per unit at standard wholesale. Volume tiers reduce this. Allergan loyalty program (BRILLIANT DISTINCTIONS / Allē Provider) provides additional reward unit credits to high-volume accounts.
- Dysport (Galderma): typically $1.20–$1.60 per "Speywood" unit (note: Dysport's units don't equate 1:1 with Botox units; the typical conversion is approximately 1 Botox unit = 2.5–3 Dysport units, so unit-cost comparison must account for the dose ratio).
- Xeomin (Merz): typically $4.50–$5.50 per unit. Free-of-protein formulation positioned as alternative for patients developing resistance.
- Daxxify (Revance): typically $5.50–$7.00 per unit. Premium pricing, longer duration (6 months claimed).
Dermal fillers (cost per 1mL syringe):
- Juvederm family (Allergan): Volbella, Vollure, Voluma, Ultra, Ultra Plus — typically $220–$320 per syringe at standard wholesale. Bundle pricing varies.
- Restylane family (Galderma): Restylane-L, Lyft, Defyne, Refyne, Kysse, Silk, Eyelight — typically $200–$300 per syringe.
- RHA family (Revance): RHA 2, 3, 4, Redensity — typically $250–$350 per syringe. Premium positioning.
- Belotero (Merz): typically $200–$280 per syringe.
- Versa (Revanesse): typically $180–$240 per syringe.
Other injectables:
- Kybella (Allergan, deoxycholic acid for submental fat): typically $400–$550 per vial.
- Sculptra (Galderma, poly-L-lactic acid): typically $350–$450 per vial.
- Radiesse (Merz, calcium hydroxylapatite): typically $250–$350 per syringe.
Account setup and volume tier structure:
Manufacturer accounts (Allergan, Galderma, Merz, Revance) require verification of practitioner license, DEA registration, and physical practice address. Approval typically takes 1–4 weeks. After approval, you move through volume tiers:
- Entry tier: standard wholesale, minimum order quantities, basic loyalty rewards - Mid tier: discounted wholesale, larger order minimums, increased loyalty rewards, account-rep relationship - Top tier: significantly discounted wholesale, master-account-level loyalty rewards, manufacturer-sponsored training and patient-acquisition support
Top-tier accounts in 2026 can realize 15–30% effective cost reduction vs. entry tier when loyalty rewards and promotional cycles are accounted for.
What patient-facing prices typically look like (markup):
- Botox: typically $11–$16+ per unit charged to patient (markup of approximately 2.5x–4x wholesale) - Dermal fillers: typically $600–$1,200+ per syringe charged to patient (markup of approximately 2.5x–4x wholesale)
Pricing varies enormously by market — Newport Beach, Manhattan, Miami Beach command higher per-unit pricing than secondary metros.
A note: pricing your services is a separate skill from your wholesale cost. Undercharging is one of the most common operational mistakes new operators make. My Practice Academy covers pricing strategy in depth — both how to set initial pricing and when to adjust as the practice matures.
